Eyeglasses/Hearing Aid Application

The Central City/Centralia Lions Club offers assistance to those individuals (adult or children) in the community with eyeglasses or hearing aids. 
 
WE WOULD LIKE TO TO ASSIST ALL THOSE WHOSE SUBMIT AN APPLICATION FOR EYEGLASSES OR HEARING AIDS, UNFORTUNATELY, WE ARE LIMITED TO ASSIST THOSE PEOPLE WHO LIVE IN ILLINOIS
WITHIN 40 MILES FROM THE
CENTRAL CITY/CENTRALIA LIONS CLUB.

If you click on the link above, you will be supplied an application that you can print out to apply for eyeglasses or hearing aids. 

        The application should be completely filled out and mailed to the address provided on the application
with ALL required documents.

Your application will be reviewed by our eyeglass committee and you will be contacted whether you qualify for free eyeglasses or hearing aids through the Lions Club.

The Central City/Centralia Lions Club offers you a chance
to apply for a $500 Scholarhship to Kaskaskia College.

 If you click on the link above, you will be supplied an application that you can print out to apply for the
Charles VanCleve Memorial Scholarship. 
The eligibility requirements are included
with the scholarship application.

     The deadline for submitting an application is JUNE 30, 2011.

     The scholarship application should be completely filled out and mailed to the address provided on the application.
